Student must:

Attend a Nevada public, charter, private, or home school

One of the following must be true:

If one of the above is true, one of the following must also be true:

Guardians income is below poverty threshold as defined by the US Department of health and human services.
Student is uninsured, income is low but is ineligible for Medicaid or other federal or state subsidized insurance program
Student is insured, but trauma specific service are not covered
Student is insured, but services are unaffordable because of high deductible
Student is insured, but services are unaffordable because the cost exceeds insurance cap
